This form is to be used by City of Takoma Park residents who will not be present at their usual city address during the November 8, 2022 City Election for mayor and city council.

The City election will be "vote by mail" and every registered voter will receive a ballot by mail in mid-October.

Ballots will be sent to the Takoma Park residential address on file unless an absentee ballot is requested.
